竞赛报名表是报名表的一部分，指定用于报名程序中符合参加奥运会资格的运动员正式报名。竞赛报名表分为“团体表格” 或“个人表格”（参见《奥林匹克宪章》第49条）

This form is a part of the Entry Forms and provides for the official entry of qualified athletes into the Olympic Games within the Entries Process. The Sport Entry Form may be ‘team’ or ‘individial’. (see Olympic Charter Rule 49)
